The PlayStation 5 version has 24 trophies: 6 bronze, 11 silver, 6 gold and 1 platinum. The Steam version has 23 achievements. The difference is the platinum, which exists only on PlayStation.
The platinum is titled “The Kind of Moment That Lasts Forever”, and it is earned by getting every other trophy. See the PS5 page for the platform details.
What the trophies cover
The sources group the trophies into story progress, boss kills, collectibles such as maps and mementos, upgrades to maximum, and a few extra actions. Collectibles are the type most easily missed, so check them as you go.

- Find all the collectable maps. The map trophy matches the 11 district maps. See the map page.
- In the tunnel, in a heartbeat. Survive the tunnel ambush.
- Killer Frequency. Defeat Deputy Roberts at the Riverside police station.
- Down the Rabbit Hole. Reach the Rabbit Hole club.
- Apartment 4B. Complete Union Apartments, which includes beating the boss in room 4B and recovering the bell.
- Meeting Adjourned. Place all the masks on the statue in the park. This is an extra-action trophy.
- Memento slot unlock. Unlocking the second Memento slot is one of the upgrade conditions.
What the sources do not confirm
The sources do not list the hidden trophies, so this page does not give a complete hidden list. They also do not name the trophies tied to particular endings. The number of playthroughs the platinum needs is not stated, so do not assume a playthrough count.
